What information or documents will I need to provide to help you file my claim?

1 - Prescription records evidencing your legal prescription of products manufactured by Mallinckrodt and its subsidiaries
2 - Treatment records for any inpatient, outpatient, and medication aided treatment such as suboxone or methadone
3 - Any medical records in which it indicates a diagnosis of addiction or opioid use disorder
4 - If it is a death claim a copy of the death certificate

How much does it cost me to file my claim?

There is no fee to file your claim, we only get paid when you get paid. Some pharmacies may try to charge a few dollars to retrieve your records but you should be able to get them for free.

How much should I expect to get for my claim?

At this point in time it is impossible to say for sure. There are so many variables involved plus it also matters to what extent your damages are. These are not determined by us but the examiners reviewing the claims.

When should I expect to get anything for my claim?

This is quite similar to the 'How much' question. There are just too many moving parts involved that we can't even guess at when the final determinations are made. We will however provide status reports on how the claims process is coming along.

What am I signing up for?

You are signing up for a creditor claim against Mallinckrodt Pharmaceuticals and their subsidiaries in their jointly administered bankruptcy. These were the producers, manufacturers and marketers of opioid medications that resulted in addiction and the opioid crisis. We are not filing a claim against any other entity or person on your behalf.

Is this part of the Purdue lawsuit?

No, these claims are for Mallinckrodt Pharmaceuticals.
